In fact, what we find in acupuncture is more like a spectrum of opinion. At one extreme there are adherents of the traditional system in its fullest form; most of these, Modern acupuncture 23 although not all, are people who have approached acupuncture without first undergoing a conventional training as a health professional. At the other end of the spectrum are those who reject the traditional system outright and who have often devised their own terminology; for example, some prefer to speak of ‘dry needling’ rather than acupuncture.
It is a matter of choice whether or not to use the tubes, but if you are dependent on them you may encounter difficulties if the only needles available happen to lack them. I don’t like to teach people to use techniques I don’t use myself and I therefore always show students how to insert the plain needles, but there is no reason why acupuncturists should not use the tubes if they wish. One advantage of using them is that the pressure of the tube appears to act as a distraction and makes the penetration of the needle almost painless.
Anatomy is obviously of particular importance but different kinds of health professionals employ anatomical knowledge to differing extents in their ordinary work. The everyday practice of clinical medicine does not always require detailed knowledge of anatomy. When performing acupuncture in a potentially hazardous anatomical region you should make sure your anatomical knowledge is fresh and adequate. If there is any doubt in your mind you should either not do acupuncture at all or else confine yourself to superficial needling no deeper than the subcutaneous tissues; this is safe and may be surprisingly effective.
